Best as I can piece together in very, very sporadic viewing. they've rolled it all together into one plot.
David (Vincent Izarry's character) was part of the 'missing Maria Santos' plot and the missing 'Greenlee' plot a couple of years ago, so they are saying that the same stunt he pulled on Greenlee and Maria is what he's done for a bunch of people in town. That he was running some strange experimental medical trial and so several of the supposed deaths aren't actually... dead.
Erica's plot is unrelated. Primary reason that they stuck her in Greenhaven was to write the returns of some of the beloved loonies, so she's in Greenhaven with Janet from Another Planet, Crazy Annie, and Stuart's wife whose name I'm forgetting.
Basically, AMC decided to just go whole hog. Screw reality, give fans the reunions they wanted. Can't say that I blame them.

Hence, Dixie isn't dead. Leo (Duhamel) isn't dead. Zach isn't dead. Gillian isn't dead.
