Spike heard the name "Halfrek" in the prologue, but he was dead drunk at the time. He never actually heard her called Halfrek on the show (at least not in Season 6. Maybe he did in Season 7). So I'm just taking it as Spike being a bit forgetful. Since she told him in the prologue he should know, it's just laziness on his part that he doesn't.
